Issuer: University of Health Sciences (UHS), Lahore. Purpose: Mandatory production of original valid identity document for entry to the Medical and Dental College Admission Test (MDCAT-2026) scheduled on 16 August 2026.

Biometric verification of candidates will be conducted at examination centres. Only the specified original documents (CNIC, JIC/Smart Card, or original valid passport where applicable) will be accepted for entry. Photocopies, scanned copies, digital images, laminated copies, expired identity documents, or any other documents will not be accepted.
Not acceptable: B-Form (Child Registration Certificate), secondary/school certificates, matric/intermediate certificates, higher secondary school certificates, domicile certificates, driving licences, or any other certificate/document for entry.
All candidates are advised to obtain the requisite valid identity document from NADRA in advance to avoid inconvenience. Failure to produce the prescribed original valid identity document at the examination centre may result in denial of entry.
