Show Cause Notice to Clinical Technician (Surgical) — Lady Reading Hospital, Peshawar

Issuer: Lady Reading Hospital (Medical Teaching Institution), Peshawar.

To: Mr. Shahzad Ali S/O Mir Zaman — Clinical Technician (Surgical) (HRID: 6-1616), LRH-MTI, Peshawar.

Purpose: Show-cause notice issued after re-verification of Surgical Technology qualifications. The Faculty of Paramedical and Allied Health Sciences reportedly declared the submitted diploma fake. The hospital states that submission of a counterfeit diploma amounts to fraud, misrepresentation and concealment of material facts, rendering the appointment void ab initio and noting continuous misconduct including unavailability for service since 2013.

Required action: Mr. Shahzad Ali is called upon to submit a written explanation within three days of receipt explaining why major penalty under section 15-3.4 of the LRH-MTI Human Resource Manual should not be imposed. The reply must be supported by documentary evidence and submitted within the stipulated time. Failure to respond may result in the matter being decided ex parte. The recipient may also request to be heard in person by the competent authority.
