Issuer: College of Physicians & Surgeons Pakistan (CPSP). Purpose: Engagement of well-reputed Architectural & Engineering consulting firms for the designing and construction supervision of the CPSP Regional Centre in Loralai, Balochistan.
Terms of Reference (TORs) can be purchased from CPSP Regional Centre Quetta or CPSP Headquarters Karachi on written request. TOR fee: Rs. 5,000/- (non-refundable). Payment by pay order/bank draft in favor of College of Physicians & Surgeons Pakistan.

| 9. Non-Blacklisting Affidavit | Affidavit on judicial stamp paper confirming the firm is not in litigation or blacklisted |
