Application fee & payment: Rs. 1,000 (non‑refundable). Apply on the prescribed HMC application form and submit with bank draft/payment order in favor of Hospital Director / MTI‑HMC or via direct deposit to Bank of Khyber, HMC Branch (Receipt Account, Branch Code 5185) as specified on the form.
