Medical Superintendent, District Headquarter Hospital Landikotal invites sealed bids for supply of medicines, medical & surgical devices, medical gases, laboratory chemicals and contingency/stationery/repair, bedding & clothing items. Undertaking on stamp paper that bidder is not blacklisted by any Government/Semi-Government institution. Successful bidder responsible for all transportation to/from the hospital; quoted prices must be inclusive of transportation and taxes.
Contingency Items, Stationery, Repair of Equipment & Furniture, Bedding & Clothing	Rs. 50,000	Bid security from bidder's bank account per KPPRA modalities. Bidder must be registered with Income/Sales Tax Department and shown as Active Tax Payer on FBR list. Undertaking on stamp paper that bidder is not blacklisted by any Government/Semi-Government institution. Samples required where items are specified as sample-based.
Laboratory Chemicals / Consumables	Rs. 50,000	Bid security from bidder's bank account per KPPRA modalities. Undertaking on stamp paper that bidder is not blacklisted by any Government/Semi-Government institution. Manufacturer/importer/distributor authorization letter must be submitted with the bid where applicable. Rates and quality will be decided by the Purchase Committee and laboratory skilled personnel.

All bidders must ensure compliance with the listed registration, tax, licensing and anti-blacklisting undertakings.
